General Anxiety Disorder Self-Assessment
The GAD-7 (General Anxiety Disorder-7) is a self-assessment tool designed to help you identify symptoms of anxiety and their severity. This survey consists of seven questions, each related to common symptoms of general anxiety experienced over the past two weeks.
Over the last 2 weeks, how often have you been bothered by any of the following problems?
Scoring Instructions:
After completing the survey, calculate your total score by adding the numbers corresponding to your answers for the first seven questions. Refer to the score interpretation below to understand the range of your score and what it may indicate about the severity of your symptoms. If your score suggests moderate to severe risk, please seek further professional assessment.
Score Interpretation:
0-4: No to low risk, no intervention required.
5-9: Mild risk level, symptoms to be monitored.
10-14: Moderate risk level, indicates possible clinically significant conditions. Further evaluation is recommended.
15+: Severe risk level, active treatment likely warranted.
